After a bunch of experimentation, I found that if I disabled PCI master burst mode in the BIOS setup, the timeout problems all went away. Disk I/O speed was -- not surprisingly -- noticeably slower with burst mode turned off, but the system was stable and reliable. I'm not sure if the primary culprit here is my Promise card or my motherboard, but it does suggest that some systems with iffy PCI bus tolerances might require suboptimal BIOS settings in order to work properly with some Promise controllers. As for whether any effective steps can be taken in the driver to make it more robust against this problem, I'll have to leave that to people who are more familiar with the hardware and the code. There was no immediate message, It appeared to me that a newfs command simply never completed, and the installation program just waited. Hitting escape caused a message to appear, but I would have to re-install to report that to you. When I print or upload the same file, panic repeated (I try 2-3 times). Almost always after panic a few files on / or /usr was damaged, fsck won't doing background preen. Seems this problem is still present. I can rule out broken/flaky hardware, because the server has been running 4.x-STABLE since my last reply and it was rock-solid under heavy load. It's nice that most people experiencing this problem can use the floppies as a workaround, but if I try that there is (lots of) data corruption when there's a little load on the system.
