Hmmm.... well, at the moment pkg(8) has nothing to do with /etc -- as that's part of the base system, it's outside pkg(8)'s ambit. For now -- there are plans to package the base system, but they are as yet only as a very early stage. However, in the more general case, pkg(8) has a plugin capability which sounds like it would fit the bill. You should be able to set up some sort of notification to etckeeper to trigger what ever action you require on package installation / update. The plugin system in pkg(8) has not been particularly widely used to date, so undoubtedly there are nits to comb out.
