We actually have two different versions of pam_ssh in addition to the port: one in src/lib/libpam/modules and one in src/crypto/openssh/pam_ssh. We used to build the latter until Brian unhooked it from the build; then Mark readded it, and now we build the former. All three versions (the port and the two in the tree) are way out of date relative to Andrew Korty's version and have different, possibly overlapping, sets of bug fixes, possibly including fixes that aren't in Andrew's version. I've been meaning to clean up this mess but haven't gotten around to it yet.
