For i386 (small instances) EC2 either needs to update to a non-antique version of Xen or support for not using linear pagetables needs to be restored to xen/pmap.c. For amd64 (medium and large instances) a new xen/pmap.c would need to be written. This isn't a great deal of work, but most users seem to be having a great deal of success with just running HVM with PV drivers.
