I had this problem, adding the following line to /etc/rc.conf has made it
better:

natd_flags="-s -m -redirect_port tcp 192.168.100.40:2000-3000
2000-3000 -redirect_port udp 192.168.100.40:2000-3000 2000-3000"

where 192.168.100.40 is my internal machine,
now set your ICQ up to use the port span 2000-3000.
