I GIVE UP!  Go look at http://people.freebsd.org/~reg/ now! 
Specifically, go look in bsd.ports.use.mk.

WITH_GTK is the offical knob.  vim5 will be included in my next sweep
through the tree looking for ports using options other than WITH_FOO or
WITHOUT_FOO, if you haven't fixed it.
