No, but I can describe the process.

1)  Backup /etc to somewhere safe.
2)  Make & boot the 2.1.5 boot floppy.
3)  At the menu, select the "upgrade" option.
4)  Follow the prompts, mounting and not newfsing your existing
partitions.
5)  Take note of which /etc files couldn't be converted.
6)  At the prompt, copy back services from your backup etc directory, and
merge your sysconfig as appropriate.  The best way to upgrade sysconfig
imho is to print out your old one and just re-fill in the new one with the
old info.
7)  Reboot, build a new kernel, install, reboot again, be happy.  ;)
