As a datapoint, I'm also seeing this with ncr.c 1.128:

	ncr0: restart (scsi reset).
	ncr0: SCSI pahse error fixup: CCB already dequeued (0xf0780400)
	ncr0: timeout nccb=... (skip)

	this goes on and on.

I've also got a worm installed. I don't know what happens if I
disconnect it from the bus. If desired I will, though... The last
working kernel detects:
