- Most are from old bonobo-activation, so I think it's too late to
   make the change? Since, it's not in the ports tree anymore and now
   it's libbonobo.

- /usr/X11R6/lib/X11/fonts/*/fonts.cache-1, the x11-fonts/XFree86-font*
   are supposed to take care of those, correct?

- ThinIce, looks like you don't have the up to date of
   x11-toolkits/thinice.

- /usr/X11R6/share/devhelp/books/gtkmm-2.0/gtkmm2.devhelp, it's exist
   in the x11-toolkits/gtk--2/pkg-plist. Perhaps, it was from old port?
   Or, need to bump the PORTREVISION if you have the up to date?

- /usr/X11R6/etc/gtk-2.0/*, same comment as above gtkmm-2.0..

I didn't check on others, but so far looks like you have out of date 
packages installed, need to bump the PORTREVISION or something else.
