I don't have any idea. I used my copy of the 4.1-iso on a W2k Server
to create both floppies, walked into the other room and stuck kern.flp
into the floppy and had the system restart. The new floppies booted
just fine. 

If your copy of the iso was messed up, I can't imagine fdimage working
from it to create the floppies. I've never seen anything like that die
silently.
