I have been very impressed with the combination of AVP for Unix with 
this hack: http://www.decros.cz/~reho/check_virus/

It does a terrific job of detecting, notifiying, and removing viruses
from the e-mail stream without adding much load to the server.

AVP has their own hack for Sendmail called avpkeeper - don't bother. Use
the hack to sendmail from Petr Rehor (above link) -- it works much better.

Cost for the Antivirus system (at least in the US) is $99.00/year.
