This is usually a hardware problem; either bad memory, a bad motherboard
or a bad disk controller have caused it for me.  You don't say whether
you're running a busmastering disk controller, if not then I'd almost
certainly say memory error.  You may want to check your bus speed 
settings as well, though I haven't had one of these that could be tracked
to that.
