Sorry.  It is a three button PS/2 mouse, an AT&T 320.  I think it is really
a Logitech mouse in Deathstar clothing.  From dmesg:
May 24 13:59:31 rally3 /kernel: psm0: <PS/2 Mouse> irq 12 on atkbdc0
May 24 13:59:31 rally3 /kernel: psm0: model Generic PS/2 mouse, device ID 0

I never had a problem with the mouse under 2.2.8, 3.x, or NT.  (I really
like these mice: 3m cords).

I also found the following lines in syslog: 
May 25 15:33:34 rally3 /kernel: psmintr: out of sync (0080 != 0000).
