Nopes.  You only need to declare in *one* of these files that you will
relay mail for some host.  I tend to prefer using access_db and my
/etc/mail/relay-domains file does not even exist, but I haven't actually
tested if it's better this way or not.  It just makes it easier for me
to have to read one file [/etc/mail/access], and see which domains are
relayed, which are blocked, etc.
