Is your modem a PNP modem ?  If it is, did you re-edit your sio.c struct with
the vendor code and put something like this in your /kernel.config file.
I mean your / directory kernel.config not your kernel config in
/usr/src/sys/i386/conf/YOUR_CONFIG_FILE.

There is a different way of doing things now in the 3.+ branch.

Here is my kernel config in /usr/src/sys/i386/conf.
I actually disable sio1 so my modem's uart can take its place.

device          sio0    at isa? port "IO_COM1" flags 0x10 tty irq 4
#device         sio1    at isa? port "IO_COM2" tty irq 3
#device         sio2    at isa? disable port "IO_COM3" tty irq 5
#device         sio3    at isa? disable port "IO_COM4" tty irq 9  

Here is my dmesg:
