Put the following to your xorg.conf:

Section "ServerFlags"
  Option "DontZap"         "off"
  Option "AllowEmptyInput" "off"
  Option "AutoAddDevices"  "off"

Section "InputDevice"
  Option "XkbOptions" "terminate:ctrl_alt_bksp"

This works for me very reliable (xorg-7.4_2).
