Could be one of several things:
Floppy disk has bad sectors -> have you tried another floppy disk?
Floppy is not 1.44MB -> make sure you're not using an old 720k floppy
_disk_
Floppy drive is a 720K -> is it a 1.44MB floppy disk _drive_
