Let me guess: this happens under 4.1.x??
I had the same problem when I was upgrading from 4.0.3 to 4.1.x and
so I installed 4.2.0 instead--I have not had a problem since...
However YMMV. The port for XFree86 4.2.0 found here:
        http://people.freebsd.org/~jmz/XFree86-4.2.0.tar.gz
