OPTi931, OPL3, or CS423x based cards. 
My $15 OPTi931 works rather well (including full-duplex). 
Only Speak Freely seems to be unhappy with it (it locks up
the machine when I try to do full duplex, even with the HALF DUPLEX
makefile option commented out.
