We saw this exact same problem after upgrading to 4.5 on a Compaq Proliant 5500 with a SMP kernel. The "fix" was to run a GENERIC kernel which does not exhibit these errors. There does appear to be a problem with the driver, but it only shows itself when we run a SMP kernel so it also appears to be related to the SMP code.
