You just boot the 386 install disk and install it, the same as you
do on any other 386 box.  Works great for me.

If you want to run some 386 applications on top of an amd64 kernel,
I agree that's a lot trickier.  If I had more time, I would work on
getting cross-compiling ports to work properly.
