The Intel two-port server cards have been workin well for us, both with
3.4-STABLE and 4.0-STABLE:

fxp0: <Intel EtherExpress Pro 10/100B Ethernet> port 0x3100-0x311f mem 0xfe800000-0xfe8fffff,0xfea00000-0xfea00fff irq 17 at device 4.0 on pci1
fxp0: Ethernet address 00:d0:b7:5f:b7:a8
fxp1: <Intel EtherExpress Pro 10/100B Ethernet> port 0x3120-0x313f mem 0xfe900000-0xfe9fffff,0xfea01000-0xfea01fff irq 18 at device 5.0 on pci1
fxp1: Ethernet address 00:d0:b7:5f:b7:a9
