This more or less finishes fully prototyping all the headers except for
ddb and i386/linux.  Expect it to cause a few (< 100 :-) warnings for
some time.  I haven't figured out what to do about warnings for protosw
function pointers.  There are few enough of them for non-bogus fixes to
be practical.
