I had this one one machine (of 4 I supped at the same time) about 2 or
3 weeks ago.

It seems some PATH was set wrongly someplace, but my quick fix was
to add "touch m4" in the following in /usr/src/Makefile.inc1:

# installworld
#
# Installs everything compiled by a 'buildworld'.
#
distributeworld installworld: installcheck
        mkdir -p ${INSTALLTMP}
        for prog in [ awk cat chflags chmod chown date echo egrep find grep \
            ln make makewhatis mtree mv perl pwd_mkdb rm sed sh sysctl \
            test true uname wc zic touch m4; do \
                cp `which $$prog` ${INSTALLTMP}; \
        done
        cd ${.CURDIR}; ${IMAKE} re${.TARGET:S/world$//}
        rm -rf ${INSTALLTMP}

I've not seen it happen since either.
