Following up on my message of last week, about not being able to get
my 4.2-STABLE system to produce a crash dump:

I finally managed to get a dump by typing "call dumpsys" at the DDB
prompt. (Thanks to Bosko Milekic for this suggestion.)

I'm still perplexed that the more conventional approaches (such as
typing "continue" at the DDB prompt) gave me a type-12 trap (page
fault in kernel mode) instead of dumping.
