I just noticed this thread and remembered I too had had problems with my
Adaptec 2940 SCSI adaptor (ahc0 <Adaptec 2940A Ultra SCSI host adapter>).
The card would not recognize the hard drive after I restarted the system
(fastboot), but it would if I powered it off and turned it back on again.

I contacted Adaptec and was sent a BIOS update (to version 1.3) that
solved the problem.
