Afaik HEAD is 1.21 and RELENG_4 is 1.15.2.1, and I can't
find
  OPTIONAL_MANPATH	/usr/X11R6/man
in HEAD:
  http://www.freebsd.org/cgi/cvsweb.cgi/src/gnu/usr.bin/man/manpath/manpath.config?rev=1.21
nor
  OPTIONAL_MANPATH	/usr/local/man
  OPTIONAL_MANPATH	/usr/X11R6/man
in RELENG_4:
  http://www.freebsd.org/cgi/cvsweb.cgi/src/gnu/usr.bin/man/manpath/manpath.config?rev=1.15.2.1

Did you try the 'How-To-Repeat' examples?
