During the installation process select the distribution set "ALL" and proceed with the installation process.  After a while I get an error message stating that it could not find the INDEX file and gives me further error message saying that it could not find certain x files and after the installation I discover that x has not been installed.  I then tried running sysisntall after the installation process, chose the selection of installation sets and encountered similar errors.  The error message was "unable to get packages/INDEX from selected media.
