I think it has been mostly caused for me in moments where heavy UUCP-over-TCP traffic went over my external slip link, and i've tried to do something else in the same second across the link (ping someone outside, rlogin to freefall etc.). The bad effect for me was that this will also predictably cause me mail loss (the UUCP jobs did already complete, but their data files on the disk are either missing [not yet backed] or totally garbled [vm or file system consistency problem]).
