    I get the same behaviour you mention in your first three points,
but not the "-- MARK --" syslog message.  This behaviour is
reproducible when using NCSA httpd 1.4 with 50+ simultaneous
connections and 15+ requests per second.  I'm not running quotas on my
system, although NFS filesystems are heavily used.
