I have seen a similar LOR with GFS when using a file-backed ZFS pool on
FreeBSD-11.0-CURRENT-amd64-20160206-r295345.=20

It's easy to reproduce:

# dd bs=3D1m count=3D256 if=3D/dev/zero of=3D/tmp/disk1
256+0 records in
256+0 records out
268435456 bytes transferred in 0.266620 secs (1006810670 bytes/sec)
# zpool create testpool /tmp/disk1
# zpool destroy testpool
