For the record, I saw this with 4.2.4 so I don't think this is 
something with the new code.  I haven't seen it yet with 4.2.6 but it 
sounds like it hasn't been resolved yet.  It doesn't occur very 
frequently as you have noticed so it is hard to troubleshoot.
