Here is what I've been running for months, with no trouble.
Fore PCA-200E, FreeBSD 3.3, HARP software:

atm show config
Intf      Vendor    Model     Media           Bus   Serial No
hfa0      Fore      PCA-200E  OC-3c           PCI   61379
    MAC address = 00:20:48:04:ef:c3
    Hardware version = 2.0.1
    Firmware version = 3.0.1

ls -l /etc/pca200e.bin
-rw-r--r--  1 root  wheel  40984 Oct 29 11:31 /etc/pca200e.bin


These are connected to Fore ASX-200BX and ASX-1000 switches.

Works great, love my ATM.  Beats the pants off our (very good) 100baseT
path.
