Using the NEWCARD config (but copied to another file named OBRIEN1 on my machine), no changes made.

The card's PCI vendor/device fields are 115d:0003, as reported.  The MAC address of the card is 00:10:A4:EC:8D:7A (although listed as 00:00:00:00:28:00), which is hinted at under one of the Function Extension lines...
