I had something like that with my pccard modem at first. Try adding
"reset 1000":
card "LUCENT-VENUS" "PCMCIA 56K DataFax"
config 0x27 "sio2" 3
reset 1000
insert echo Lucent-Venus Modem inserted
insert echo Lucent-Venus Moden removed
