It seems you missed the "make install" command.  This will fail if the
apache daemon is still running. Thus, an "apachectl stop" or
"/usr/local/etc/rc.d/apache stop" has to be done first.

This confirms nothing has been installed.

Usually mod_php4 asks which options should be compiled in. Have you
checked the gd button at this screen? If so, have you checked the output
of make for errors?

/usr/local/etc/httpd.conf should contain something like
"LoadModule php4_module        libexec/apache/libphp4.so"
"AddModule mod_php4.c"
"AddType application/x-httpd-php .php"
at various places. This should be added automatically.

php.ini overrides compiled-in defaults. If it is not present, you're
using the defaults.
