For RELENG_4, I used the same patch that is used on RELENG_5 and RELENG_6. ie

ftp://ftp.FreeBSD.org/pub/FreeBSD/CERT/patches/SA-06:13/sendmail.patch

and it seems to apply cleanly and work.
