Try:

   device       sbxvi0   at isa? drq 5
   device       sbmidi0  at isa? port 0x330
   device       opl0     at isa? port 0x388

Works for me in 4.2-RELEASE at least, albeit with a different card (Sound
Blaster 32, which has a Vibra 16 DSP I believe).  Check pcm(4)'s flags docs
if you go with PCM.
