I fixed this problem a while ago, and the konsole maintainers have a
patch.

Basically, the problem is that the file descriptor used to pass the PTY
to konsole_grantpty is 3, but libc_r uses file descriptor 3 internally. 
This causes the dup2() in TEPty.c to fail (which simply does a quiet
exit(1), without printing an error...).

You can simply change a #define to fix the problem, but there's still a
race condition there, so you're not really secure. A revoke() at the end
of konsole_grantpty.c is also required...
