The solution.

The 41upgrade.tgz kit installs new versions of pkg_add,
pkg_delete etc utilities used by the ports mechanism. The new
utilities require the libfetch.so.2 library which is not
available on 4.0-RELEASE. Solution: find somewhere a 4.2-RELEASE
or STABLE system, copy the binary libfetch.so.2 into /usr/lib,
then 'ldconfig -R' and enjoy.
