Sorry to follow-up to my own posting, but I've come across a number of tools
which look promising.  Most interesting is the one described here:

  http://www.helpmaster.com/help/other/popudo.htm

It is shareware :-(, but has a Linux version (that may run under emulation),
and can apparently read and write a plethora of file formats, including
WinHelp.
