OK.  It looks like the card isn't being noticed on insertion.

cbb0: bad Vcc request. ctrl=0xffffff88, status=0xffffffff

This looks like cbb0 is getting a bad address.  I should have had you
include a devinfo -r as well.  Please include the whole thing since
there may be important clues in the non-cbb0 devices.
