I use the RE-100, the Ethernet only version of the card, with no
problem. Can you send a dmesg of the boot. Specifically the pcic
and  pccard information is of some interest. There are a few bridge
devices that are known to have problems.

Best guess: try setting the following sysctl:
sysctl -w machdep.pccard.mem_start=0xd0000

If it works, put it into /etc/sysctl.conf.

If not, try 0xd4000 and 0xd8000.
