Do you have something like the following in /var/qmail/rc,
or any other file you start qmail with?

/usr/local/bin/tcpserver -v -H -lhostname -u 82 -g 81 \
    -x /var/qmail/tcp.smtp.cdb 0 smtp /var/qmail/bin/qmail-smtpd &

This starts tcpserver on smtp port.  When a connection
is accepted, it is passed on to qmail-smtpd.  Please
check the options for tcpserver, you probably don't
want some of the options I have.
