It's an SMC Elite Ultra 16. 

Normal setup is irq 5, ioaddr 0x280, memaddr 0xD8000

Per /usr/src/sys/i386/config/GENERIC
device ed0 at isa? port 0x280 net irq  5 iomem 0xd8000 vector edintr
device ed1 at isa? port 0x300 net irq  5 iomem 0xd8000 vector edintr

But since you hard jumpered it and used the userconfig to match it should
work.

What messages did you get?

It should just work once configure at those parameters.
You could try using the soft setup and SMC setup disk.

You can download the latest setup disk from SMC's Web page http://www.smc.com.

Nope, that's the 8416 EtherEZ idenitied by the 83C795 chip.
