Yikes, it works! While I was trying to produce an isdn.trace file
it suddenly magically worked.

Maybe rebooting was the cure. It seemed that restarting isdnd
wasn't sufficient when applying changes to the /etc/isdn/isdnd.rc file.

Hmm. I thought entries that aren't triggered (remote-phone-incoming=5678)
can't do any harm. OK, for the future I will remind :-)
