The channel 0 slave position appears to be the most successful.  Try
moving it there and see what you get.

atapi.flp was needed for 2.1-RELEASE.  2.1.5-RELEASE does not require (or
have) an atapi.flp. 

atapi.flp IS the boot floppy image, not a combination of boot and atapi.
