Indeed, now that it has been pointed out to me that "postcode" (a
Briticism?) means postal code, aka "ZIP code" in American parlance, and
that "Switch" is a credit card company (never heard of them before), the
form starts to positively make sense.

Amazing how a sum of small things can throw you off.
