The problem was caused by a permission change in the mysql data directory, in my case /disk02/db/mysql/DATA - why and how is beyond my comprehension. It would be a good tactic for developers to to include a troubleshooting script to check directory permissions in installations, as this is such a common problem. At a minimum a simple checklist would help. I have had situations where I have looked at files/directories after an installation and could not tell what it should be.
