Sorry, I just realised I didn't actually answer this part of the question.

These Mylex adapters aren't really "SCSI" controllers at all, they just 
look a bit like one.  In the case above, the driver has received 
three commands back from the controller which have illegal "slot 
numbers".  The "slot number" is just a tag the driver uses to identify 
the command; we only issue slot numbers from 1 up to around 256, so the
presence of those higher numbers indicates that something has gone wrong;
either the controller firmware has crashed, or something else has 
trampled over the status ring.
